Where is the best place to keep one's passport when touring the city? On your person or in the hotel's safe? Any additional tips about passport safety will be greatly appreciated!

I laminate a copy and use that when I need ID (at internet cafes, registering at hotels, renting audio guides, etc) and keep my original locked in my suitcase in the room. Never had a problem.

My experience has been that most hotels in Europe provide a safe in the room and I leave my passport in it along with my additional credit cards and cash that I don't think I'll need that day. For identification purposes (except when registering at hotels) I always use my drivers license. The disadvantage of leaving it in the suitcase is that someone may either: break the lock; or, take the suitcase.
